• 2016 BLACKHAWK HOCKEY MEMORIAL GOLF TOURNAMENT

  Friday June 24th, 2016 - 2pm Shotgun start

  Pheasant Hills Golf Course, Hammond,Wi

  Entry Fee — $75 per golfer — includes dinner and “Desserts by Juneau”

  Mulligans—$10 per mulligan (maximum of 2 mulligans per golfer/8 per team)

  Dinner Only— $7 Per adult/$5 for kids

   

  Sponsorship— Thank you for supporting youth hockey. Remember that Blackhawk Hockey is a 501(c)(3).

  Tournament Sponsor— ($750) Includes foursome, flyer recognition, and hole signage

  Hole Sponsor — ($150) Includes hole signage

  Donations also needed for Silent Auction, Door Prizes, and Contest Winners
